There are two ways to join the Herbalcure family today!
1) Apply in person at Herbalcure corporate headquarters:
It is imperative that you provide to Herbalcure the original copy of your doctor's letter of recommendation and government issued identification during regular business hours. Your membership will be activated once your letter of recommendation and identification is confirmed.
2) Apply via fax before you come for your onsite visit to Herbalcure:
Fax a copy of your doctor's letter of recommendation and government issued identification in advance of your first Herbalcure visit at (310) 312-5203. You can be sure that your letter has been verified before you arrive. You can also call Herbalcure at (310) 312-5215 or email at admin@herbalcurecorp.com to confirm verification.
Online Form Registration - Please complete this form online, print, and fax to Herbalcure headquarters at (310) 312-5203. Include a copy of your identification and your physician recommendation.
Please be advised that Herbalcure will only allow qualified patients, caregivers or staff to enter our corporate headquarters. If you choose not to be a member, but would like to make a donation, please make all checks payable to Herbalcure Corporation.
